Please complete the audit before flu season starts
The 2026/27 Pharmacy Quality Scheme (PQS) requires all contractors completing Domain 2: Professional Practice to carry out a clinical audit reviewing the quality of Pharmacy First clinical pathways consultations. These consultations form a major part of community pharmacy practice, with pharmacists assessing patients, screening for red flags, making treatment decisions and providing safety‑netting advice.
As clinical responsibility expands—particularly with independent prescribing becoming standard from 2026—the audit supports professional assurance, competence, and consistent high‑quality clinical record‑keeping. It combines record review, structured reflection, and a peer discussion with a buddy pharmacist.
The audit examines whether consultation records clearly document:
Why the patient met the pathway criteria
What was found during assessment
The rationale for the treatment decision
The safety‑netting advice provided
It also measures how often safeguarding concerns were recorded.
Pharmacies must review a minimum of 10 unique patient consultations undertaken between 1 April 2026 and 31 March 2027, completed prospectively or retrospectively. Data is submitted via Manage Your Service (MYS), and the peer discussion must be completed by 31 March 2027.
The completed audit form (Parts A & C) is submitted to NHS England; Part B (reflection and peer discussion) is retained in the pharmacy until 31 March 2030.
To evaluate and improve the quality, completeness, and consistency of clinical records for Pharmacy First consultations, ensuring they meet defined standards, support safe clinical practice, and strengthen professional reflection and peer learning.

Update: Speculative GDPR breach claims targeting pharmacies (15th April 2026)
We are aware of emails and messages being sent to pharmacies that claim a data security breach has occurred, sometimes suggesting that the pharmacy is at risk of investigation or legal action.
In some cases, these messages purport to come from patients or members of the public who allege that personal data was submitted online or mishandled. However, there have been examples where the individual has had no genuine prior interaction with the pharmacy. Some approaches may be linked to speculative or “no win, no fee” activity.
Pharmacies should remain cautious, verify the source of any such messages, and avoid sharing information unless they are confident the request is legitimate. Where appropriate, LPCs may be able to advise if they have seen similar examples locally. Suspicious messages can be marked as spam to help reduce similar campaigns over time.
This should be distinguished from genuine information governance enquiries from real patients, which should continue to be handled appropriately and professionally in line with existing practice.
Community pharmacy owners are encouraged to continue working through their Data Security and Protection Toolkit. This supports pharmacy teams to review and confirm appropriate data security arrangements and the handling of information. The Toolkit must be submitted by the end of June 2026, but there are clear benefits to com
